One hearing aid vs. two

If you have hearing loss in both ears, wearing two hearing aids almost always produces better outcomes than one. Here's the science behind binaural fitting — and the situations where one aid makes sense.

How the brain uses two ears

Your brain is designed to process sound from two ears simultaneously. It uses tiny differences in timing and volume between the two ears to locate sounds in space, separate speech from noise, and reduce listening effort. This is called binaural processing, and it's one of the most sophisticated things your auditory system does.

When one ear has untreated hearing loss, the brain loses this binaural advantage. Speech understanding in noise deteriorates. Sound localization becomes unreliable. Listening fatigue increases. Wearing two hearing aids restores the binaural signal and allows the brain to work the way it was designed to.

Benefits of two hearing aids (binaural fitting)

  • Better speech in noise — the brain can use the "squelch effect" to separate speech from background noise when it has input from both ears
  • Sound localization — knowing where sounds are coming from (critical for safety — traffic, alarms)
  • Reduced listening effort — less mental fatigue at the end of the day
  • Balanced hearing — sounds come from all directions, not just one side
  • Prevention of auditory deprivation — an unaided ear can lose speech discrimination ability over time; keeping both ears stimulated preserves function
  • Binaural processing features — modern hearing aids communicate wirelessly between ears to coordinate processing, a feature that only works with two aids

When one hearing aid is appropriate

One hearing aid is the right choice in specific situations:

  • Unilateral hearing loss — hearing loss in only one ear; the other ear is normal or near-normal
  • Single-sided deafness (SSD) — one ear has profound loss; a CROS or BiCROS system (a specialized type of hearing aid) may be more appropriate than a standard aid
  • Asymmetric loss where one ear doesn't benefit from amplification — rare, but assessed by audiologist
  • Budget constraints — one aid is better than none; start with the worse ear

The auditory deprivation effect

Research shows that when one ear with hearing loss goes unaided for years, the brain gradually loses its ability to process speech from that ear — even after a hearing aid is fitted. This is called auditory deprivation, and it's one of the strongest arguments for fitting both ears as early as possible.

The effect is reversible in the early stages but becomes harder to reverse over time. If your audiologist recommends two aids, this is one of the key reasons why.

Cost considerations

Hearing aids are priced per pair, but some providers sell them individually. If budget is a genuine constraint, discuss this with your audiologist — they may recommend starting with one aid for the worse ear and adding the second later. However, be aware that some binaural processing features (ear-to-ear communication, coordinated noise reduction) only work when both aids are present.
