Dementia and Hearing Loss: The Connection Caregivers Need to Know

The research is stronger than most people realize

For years, hearing loss and cognitive decline were treated as parallel but unrelated consequences of aging. That view has changed substantially. A growing body of research — including a landmark 2020 Lancet Commission report and multiple large-scale longitudinal studies — now identifies untreated hearing loss as one of the most significant modifiable risk factors for dementia.

The 2020 Lancet Commission on dementia prevention, intervention, and care identified 12 modifiable risk factors that together account for approximately 40% of dementia cases worldwide. Hearing loss was identified as the single largest modifiable risk factor, accounting for an estimated 8% of dementia cases — more than physical inactivity, smoking, depression, or social isolation.

A 2023 randomized controlled trial (the ACHIEVE study, published in The Lancet) found that hearing intervention reduced cognitive decline by 48% over three years in older adults at elevated risk for dementia. This is the strongest evidence yet that treating hearing loss — not just identifying it — may meaningfully slow cognitive aging.

Why hearing loss affects cognition

Researchers have proposed several mechanisms:

Cognitive load: When hearing is impaired, the brain must work harder to decode speech and sound. This increased cognitive effort — sometimes called "listening effort" — draws on cognitive resources that would otherwise be available for memory, attention, and executive function. Over time, this chronic cognitive overload may accelerate decline.

Social isolation: Hearing loss makes conversation difficult and exhausting, leading many people to withdraw from social engagement. Social isolation is itself a well-established risk factor for cognitive decline and dementia.

Brain structure changes: Some research suggests that untreated hearing loss is associated with accelerated brain atrophy, particularly in areas involved in speech processing and memory. Whether this is a cause or consequence of cognitive decline is still being studied.

Shared pathology: Hearing loss and dementia may share underlying biological mechanisms — vascular disease, inflammation, or neurodegeneration — that affect both the auditory system and the brain simultaneously.

What this means for caregivers

Prioritize hearing evaluation

If you're caring for an older adult who hasn't had a hearing evaluation recently, getting one should be a priority — not just for communication quality, but as a potential intervention for cognitive health. The ACHIEVE study found the greatest cognitive benefit in adults at elevated risk for dementia, suggesting that hearing intervention may be most important for exactly the people caregivers are most concerned about.

Treat hearing loss as a health issue, not a convenience issue

Many families treat hearing aids as optional — something that would be nice but isn't urgent. The research suggests this framing is wrong. Untreated hearing loss in an older adult with cognitive concerns should be treated with the same urgency as other modifiable health risks.

Understand the hearing-dementia overlap in diagnosis

Hearing loss and dementia can be difficult to distinguish in clinical settings. An older adult who doesn't respond appropriately, seems confused, or has difficulty following instructions may be experiencing hearing loss rather than — or in addition to — cognitive impairment. Hearing loss is frequently underdiagnosed in people with dementia because the symptoms overlap.

If a family member has been diagnosed with mild cognitive impairment or early dementia, ensure they've had a recent comprehensive hearing evaluation. Treating hearing loss won't reverse dementia, but it may improve communication, reduce confusion, and slow further decline.

Hearing aids in dementia care

Using hearing aids becomes more challenging as dementia progresses — the person may forget to wear them, have difficulty inserting them, or remove them and lose them. Practical strategies:

  • Establish a routine: Insert hearing aids at the same time every morning as part of a consistent daily routine. Routine is more robust to cognitive impairment than instruction.
  • Use rechargeable devices: Eliminate the battery-handling challenge entirely. Place the charger in a consistent, visible location.
  • Label the case: A clearly labeled case ("hearing aids — wear every day") can serve as a reminder for someone with mild impairment.
  • Involve paid caregivers: If a home health aide or care facility staff are involved, brief them on the hearing aids — how to insert them, how to charge them, and why consistent wear matters.
  • Consider a hearing aid clip or retention cord: Reduces the risk of loss during removal.
  • Communication strategies for caregivers

    Even with hearing aids, communication with someone who has both hearing loss and cognitive impairment requires adaptation:

  • Face the person directly and make eye contact before speaking
  • Speak clearly and at a moderate pace — don't shout
  • Reduce background noise (turn off the TV, move to a quieter room)
  • Use short, simple sentences rather than complex instructions
  • Confirm understanding by asking the person to repeat back key information
  • Use visual cues and gestures to supplement speech
  • The bottom line for caregivers

    The evidence connecting hearing loss to cognitive decline is now strong enough that treating hearing loss should be considered part of a comprehensive dementia prevention and care strategy. If you're caring for an older adult — whether or not they've shown signs of cognitive decline — a hearing evaluation and, if indicated, hearing aids are among the highest-value health interventions available.
