What Bluetooth does in a hearing aid
Bluetooth in a hearing aid serves two distinct functions: audio streaming and app control.
Audio streaming sends sound directly from a paired device — your iPhone, Android phone, TV, or computer — into your hearing aids. The audio bypasses the microphones entirely, so you hear it with the full benefit of your hearing aid's processing, without room noise interfering. Phone calls become dramatically clearer. Music sounds richer. TV is easier to follow without turning up the volume for everyone else.
App control uses Bluetooth to connect your hearing aids to a companion smartphone app. The app becomes a remote control: adjust volume, switch between listening programs, fine-tune bass and treble, and in some cases, access remote support from your audiologist.
Bluetooth standards: what the specs mean
Classic Bluetooth (2.4 GHz)
Older hearing aids used a proprietary 2.4 GHz wireless protocol that required a streaming accessory (a small device worn around the neck or clipped to clothing) to connect to phones. This is now largely obsolete.
Made for iPhone (MFi)
Apple's proprietary protocol allowed hearing aids to connect directly to iPhones without an accessory — but only iPhones. Many hearing aids still support MFi for Apple users.
ASHA (Audio Streaming for Hearing Aids)
Google's protocol for direct streaming to Android phones. Supported by most modern hearing aids alongside MFi.
Bluetooth 5.x (Universal)
Phonak's SWORD chip and similar implementations use standard Bluetooth 5.x to connect to any device — iPhone, Android, Windows, Mac — without proprietary protocols. This is the most versatile option.
Bluetooth LE Audio
The newest standard, supported by ReSound Nexia and a growing number of aids. LE Audio uses less power, supports multi-stream audio (both ears simultaneously from one source), and enables Auracast — a broadcast standard that lets public venues (airports, theaters, churches) stream audio directly to hearing aids.
What to look for when comparing Bluetooth hearing aids
- Universal vs. proprietary — does it work with your phone (iPhone or Android)?
- Direct streaming vs. accessory required — modern aids stream directly; older ones need a streamer device
- Battery impact — Bluetooth streaming drains battery faster; rechargeable aids handle this better
- App quality — the companion app varies enormously between brands; read reviews
- LE Audio / Auracast — future-proofing for public venue streaming
Bluetooth in OTC hearing aids
Most OTC hearing aids include Bluetooth, though the implementation varies. Jabra Enhance Plus streams audio and uses the Jabra Sound+ app for self-fitting. Lexie Lumen includes Bluetooth streaming. Sony CRE-10 and Audien Atom Pro do not include Bluetooth — they're designed for simplicity over connectivity.
Does Bluetooth affect sound quality?
When streaming, Bluetooth audio quality depends on the codec used. Most hearing aids use standard SBC or AAC codecs — adequate for speech and acceptable for music. The bigger quality factor is the hearing aid's own processing: a well-fitted premium aid will make streamed audio sound better than a budget aid, regardless of the Bluetooth standard.
