Age-related hearing loss
Presbycusis — the gradual hearing loss that comes with aging — is the most common cause of hearing loss in adults. It affects 1 in 3 people over 65 and half of all adults over 75. It is not inevitable, but it is extremely common.

What is presbycusis?
Presbycusis (from the Greek for "old hearing") is the gradual, bilateral (both ears) sensorineural hearing loss that occurs as part of the aging process. It typically begins in the high frequencies — the ability to hear high-pitched sounds like birds, doorbells, and consonants like "s," "f," and "th" — and progresses slowly over decades.
Unlike sudden hearing loss, presbycusis is so gradual that many people do not notice it until it has progressed significantly. Family members often notice the problem before the person with hearing loss does.
Why does hearing decline with age?
Several age-related changes contribute to presbycusis:
- Hair cell loss: The cochlea's sensory hair cells gradually die over time and cannot regenerate. This is the primary mechanism of presbycusis.
- Stria vascularis atrophy: The stria vascularis is the "battery" of the cochlea, maintaining the electrochemical environment needed for hearing. It deteriorates with age.
- Reduced blood supply: Circulation to the inner ear decreases with age, depriving hair cells of oxygen and nutrients.
- Neural changes: The number of auditory nerve fibers decreases, and central auditory processing speed slows.
- Stiffening of the basilar membrane: The membrane that vibrates in response to sound becomes less flexible.
These changes are compounded by a lifetime of noise exposure, genetic factors, cardiovascular disease, diabetes, and ototoxic medications — which is why presbycusis varies so widely between individuals.
Symptoms
- Difficulty understanding speech, especially in noisy environments
- High-pitched sounds (birds, doorbells, alarms) becoming harder to hear
- Voices sounding muffled or unclear
- Difficulty hearing consonants — words sound similar ("cat" vs. "bat")
- Needing the TV louder than others prefer
- Tinnitus (ringing or buzzing in the ears)
- Asking people to repeat themselves frequently
- Difficulty hearing on the phone
Presbycusis and cognitive health
The connection between age-related hearing loss and cognitive decline is one of the most important findings in hearing research over the past decade. Multiple large studies have found that:
- Mild hearing loss doubles the risk of dementia
- Moderate hearing loss triples the risk
- Severe hearing loss is associated with a 5x higher risk
Critically, a landmark 2023 randomized controlled trial (the ACHIEVE study) found that hearing aid use in older adults at elevated risk of cognitive decline significantly slowed cognitive decline over three years. This is the strongest evidence yet that treating hearing loss may protect brain health.
Treatment
Presbycusis cannot be reversed — the hair cell loss is permanent. However, it is highly treatable:
Hearing aids
Hearing aids are the primary treatment for presbycusis. Modern devices are remarkably effective at compensating for age-related high-frequency loss. Features particularly useful for older adults include:
- Rechargeable batteries (no fiddling with tiny batteries)
- Bluetooth streaming from phones and TVs
- AI-powered speech enhancement in noise
- Telecoil (T-coil) for use with hearing loops in theaters and churches
- Companion apps for easy volume and program adjustment
Cochlear implants
For people with severe to profound presbycusis who do not benefit sufficiently from hearing aids, cochlear implants are an increasingly common option — even for adults in their 70s, 80s, and beyond.
Assistive listening devices
Captioned telephones, TV amplifiers, alerting devices (vibrating alarm clocks, flashing doorbells), and FM systems can supplement hearing aids in specific situations.
Prevention and slowing progression
While aging itself cannot be stopped, several factors can slow the progression of age-related hearing loss:
- Hearing protection: Avoiding or protecting against noise exposure throughout life
- Cardiovascular health: Managing blood pressure, diabetes, and cholesterol — all of which affect cochlear blood supply
- Avoiding ototoxic medications when alternatives exist
- Not smoking: Smoking is associated with accelerated hearing loss
