Sensorineural hearing loss
Sensorineural hearing loss (SNHL) is the most common type of permanent hearing loss, accounting for about 90% of all hearing loss in adults. It results from damage to the sensory hair cells in the cochlea or to the auditory nerve.

What causes sensorineural hearing loss?
The cochlea contains approximately 15,000–20,000 tiny hair cells that convert sound vibrations into electrical signals. Unlike the cells in your skin or liver, these hair cells cannot regenerate once damaged. Any injury to them — or to the auditory nerve that carries signals to the brain — results in sensorineural hearing loss.
Common causes include:
- Aging (presbycusis): The most common cause. Hair cells gradually deteriorate over decades, typically starting with high-frequency sounds.
- Noise exposure: Prolonged exposure to sounds above 85 dB, or brief exposure to extremely loud sounds (gunshots, explosions), destroys hair cells permanently.
- Genetics: Hereditary factors can cause SNHL at birth or cause it to develop over time.
- Viral infections: Mumps, measles, meningitis, and cytomegalovirus (CMV) can damage the cochlea.
- Ototoxic medications: Certain antibiotics (aminoglycosides), chemotherapy drugs (cisplatin), and loop diuretics can damage hair cells.
- Meniere's disease: A disorder of the inner ear causing episodes of vertigo, tinnitus, and fluctuating hearing loss.
- Acoustic neuroma: A benign tumor on the auditory nerve that can cause progressive one-sided hearing loss.
- Head trauma: Injury to the skull or inner ear structures.
Symptoms
Sensorineural hearing loss typically affects the ability to hear high-pitched sounds first. Common symptoms include:
- Difficulty understanding speech, especially in noisy environments
- Voices sounding muffled or unclear
- Trouble hearing high-pitched sounds like birds, doorbells, or children's voices
- Needing to turn up the TV volume
- Tinnitus (ringing, buzzing, or hissing in the ears)
- Difficulty hearing on the phone
- Asking people to repeat themselves frequently
Diagnosis
SNHL is diagnosed through a comprehensive audiological evaluation. An audiologist will conduct a pure-tone audiogram — a test that measures your hearing threshold at multiple frequencies — along with speech discrimination testing to assess how well you understand words. The results are plotted on an audiogram, which shows the type, degree, and configuration of your hearing loss.
Additional tests may include tympanometry (to rule out middle ear problems), otoacoustic emissions (OAE) testing, and auditory brainstem response (ABR) testing.
Is sensorineural hearing loss permanent?
In most cases, yes. Because cochlear hair cells cannot regenerate, the hearing loss caused by their damage is permanent. However, there is one important exception: sudden sensorineural hearing loss (SSHL), which occurs rapidly over 72 hours or less, may be partially or fully reversible if treated promptly with corticosteroids. SSHL is a medical emergency — seek care immediately.
Researchers are actively investigating hair cell regeneration therapies, gene therapy, and other approaches that may one day restore hearing. As of 2026, these remain experimental.
Treatment options
Hearing aids
For most people with SNHL, hearing aids are the primary treatment. Modern hearing aids amplify and process sound digitally, compensating for the specific frequencies where your hearing is reduced. They are highly effective for mild to severe SNHL.
Cochlear implants
For people with severe to profound SNHL who do not benefit sufficiently from hearing aids, cochlear implants bypass the damaged hair cells entirely and directly stimulate the auditory nerve with electrical signals. They require surgery and a rehabilitation period but can restore significant hearing function.
Bone-anchored hearing systems
For single-sided deafness or certain types of SNHL, bone-anchored hearing aids (BAHA) transmit sound vibrations through the skull bone directly to the cochlea, bypassing the outer and middle ear.
Living with sensorineural hearing loss
With appropriate treatment, most people with SNHL can communicate effectively and maintain a high quality of life. Hearing aids have advanced dramatically — modern devices offer Bluetooth streaming, AI-powered noise reduction, rechargeable batteries, and discreet designs. Audiological rehabilitation, including auditory training and communication strategies, can further improve outcomes.
